High voltage or SWER line isolation form

This form is for customers with a high voltage (HV) or single wire earth return (SWER) line connection. It is used when the HV power supply needs to be isolated so electrical work can be carried out safely.

High voltage isolations applications associated with prescribed works should be submitted via mySupply.

Important things to know before you start

We require a minimum of four weeks’ notice to be able to fulfill your request
We'll accept long lead time applications up to six months in advance
Payment must be made before the isolation date
We'll need a detailed description of works
Requested dates are subject to network availability
